12/3/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Laylah upon arrival to the home setting. Upon arrival to the home setting, Laylah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Laylah was provided the opportunity to discuss with QMHP-C any updates since the last session. She verbalized to QMHP-C she struggles with being able to communicate her feelings on a daily basis. She reports when she cannot communicate her feelings she will shut down with behaviors last 30 minutes in duration. 2. Behavior: Laylah participated in the session and Laylah did not need any redirection from QMHP-C. Mother reports she struggles with Laylah not communicating when she is mad causing tenson in the home between others with behaviors last 30 minutes to an hour in duration. 1. Intervention: QMHP-C began the session with Laylah by identifying healthier ways for Laylah to interact with peers and implementing positive decisions. QMHP-C modeled effective ways to communicate and problem solve before acting out with aggressive and disruptive behaviors. QMHP-C discussed and explored with Laylah, her feelings and thoughts as it relates to her teasing her peers within the home and community setting as it was communicated with counselor. 2. Intervention: QMHP-C explained more age-appropriate behaviors such as being respectful, accepting criticism without overreacting, and putting on her listening ears when being spoken to by an adult. QMHP-C engaged Laylah in problem solving and conflict resolutions skills and encouraged him to implement these skills by making better judgments. 1. Response: Laylah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Laylah struggles with making healthier decisions for herself. Laylah reports that she fails to think before she reacts with negative behaviors whether in the home, school and community settings. 2. Response: Laylah shared that she dislikes some of her peers and she is not going to treat them with respect if they donât treat her with respect. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Laylah continues to struggle with communicating her feelings, thoughts, and emotions in an appropriate manner. QMHP-C Plan of Action: VACCD staff will continue to use calming techniques, modeling and role-playing to teach Laylah assertive communication to reduce anger outburst.
